About MDC PCN

MDC PCN formed in 2019 and comprises of 2 general practices in Mottingham, Downham and Chislehurst areas of Bromley Borough – The Chislehurst Partnership and The Links Medical Practice.

The practices have a combined geographic area made up of around 35,000 patients.

There is a wide socio-economic and demographic served within this area which will provide an interesting and varied case mix. We pride ourselves on offering our patients the best care possible by working in collaboration with our allied health care professionals across the network. MDC PCN support in-house training of our staff. By working together, we are able to offer our services at times that may be more flexible for our patients across the PCN.

What can you expect from your PCN

Additional healthcare professionals have been recruited to work on behalf on Primary Care Networks. New roles include Care coordinators, Clinical Pharmacists, First Contact Physiotherapists, Mental Health professionals, Community Paramedics, Physician Associates and Social Prescribing Link Workers. By introducing a greater skill mix, local people will get more access to the support they need to keep people healthy and independent. 

What is a Primary Care Network?

A primary care network is a group of practices who work together to focus on local patient care. They’ve been around since 2019 and 99 per cent of practices across England now work as part of one.

With increased demand and a shortage of clinical staff, practices find they can provide greater, more personalised care for patients when they can work together on certain services or issues.
